PROCESS

 An initial evaluation will be performed together with the patient and family. If required, information will be collected from others involved in the care; e.g. doctor, nurses, therapist, dietitians and other professionals.
A care plan will be established and the care will be supervised by an experienced nurse or therapist. On-going education and information to family members is part of the program.
